Good day Adrian,

A packet capture might help, but might require some sifting through to
determine which sector accesses are in regards to the file itself and
which are for FAT processing overhead.

A bonus would be if you could also determine which sector your test file
winds up beginning on.  That could require some FAT FS info. dumping
tool, though.

You could also try enabling debugging output for those gPXE modules
involved.

# make DEBUG=int13,aoe,ata,scsi bin/gpxe.lkrn

(or whatever your gPXE build target is.)
